the Unified state register of legal entities, individuals – entrepreneurs and community groups (more – the Unified state register) – unified state information system ensuring the collection, accumulation, handling, protection, accounting and provision of information on legal and physical entities – entrepreneurs and community groups, which have no status of a legal entity. Unified state registry is created to ensure that state bodies and bodies of local self-government and of participants of civil turnover with reliable information on legal entities, social formations, not having the status of legal entities and individuals – entrepreneurs from the Unified state register. The applicant is solely responsible for the information in the usr in accordance with article 35 of the Law of Ukraine "On state registration of legal entities, individuals-entrepreneurs and community groups". For the registration of changes in information about natural person-entrepreneur, contained in the Unified state registry must submit the following documents: 1) application for state registration of changes in information about an individual – the entrepreneur, contained in the Unified state register (form No. 11); 2) proof of payment of administration fee – in the cases stipulated by this Law; 3) copy of certificate of change of registration number of the registration card – in case of any change associated with the change of the registration number of the payer of taxes; 4) copy of passport first page and page with a mark about the right to make any payments on series and number of passport – in case of any change related to the change of series and number of passport – for individuals, who through their religious beliefs refused adoption of registration number of the payer of taxes, has notified the relevant Supervisory authority and have a mark in the passport about the right to make payments for a series and passport number. If the documents are submitted personally, the applicant presents a passport of citizen of Ukraine or a temporary certificate of citizen of Ukraine or a foreigner's passport, or certificate of a stateless person, or permanent or temporary residence. In case of submission of documents, unless information on the powers of this representative is contained in the Unified state register of representative are served with copy of original (notarized copy) of the document certifying its powers. For the state registration of amendments to the data on surname, name, patronymic or location of a natural person – entrepreneur is paid an administrative fee in the amount of 0.1 minimum wage. State registration of changes in information in a shorter period held exclusively at the request of the applicant in the case of payment of the additional administrative collection fees: double the amount of the administrative fee for state registration of changes in information within six hours after receipt of the documents; five times the amount of the administrative fee for state registration of changes in information within two hours after receipt of the documents. An administrative fee will be charged the relevant minimum wage in the monthly amount established by law as of 01 January of the calendar year in which the relevant documents are submitted for registration, and is rounded to the nearest 10 hryvnia. Results of rendering of state services in the sphere of state registration (including an extract from the Unified state register of legal entities, individuals – entrepreneurs and community groups) is posted on the portal of e-services and are available for matchcode access. For submission of documents for registration of amendments to the data on natural person-entrepreneur, contained in the Unified state register, the applicant has the discretion to contact the Center of administrative services of the Chernivtsi city Council (m.
