Adoption is considered the preferred form in Ukraine the children-orphans and children deprived of parental care. For individual families this is the only chance on unique daily of the joy associated with raising kids. The inhabitants of the region wishing to adopt a child, can make a written statement about the withdrawal of registration of the prospective adoptive parents in the service for children at the place of residence. About the detailed step-by-step adoption process informs the head of regional service for children of Ternopil regional state administration Rostislav Thrush: STEP 1: Parents who wish to adopt a child, file a written application to the Service for children at the residence. The application shall be attached the following documents: • copy of passport; • certificate of salary for the last six months or copy of income Declaration for the previous calendar year, certified in the prescribed manner; • copy of marriage certificate if the applicants are married; • report on the state of health of each applicant; • notarized written consent to the adoption of the other spouse (in the case of adoption of child by one of spouses), unless otherwise provided by law; • information about the presence or absence of criminal record, issued by internal Affairs bodies at the place of residence for each applicant; • a copy of the document confirming the right of ownership or use of premises. STEP 2: After submission by the candidate within ten working days of the employee Services for children: • conducts interviews with the Complainant, ascertain the motives of adoption; • is the act of survey of living conditions of prospective parents-adoptive parents; • considering the possibility of adoption of the child; • prepares an appropriate conclusion; • in the case of a positive decision puts them on the register of prospective adoptive parents. The statement is made in the Ukrainian language, in it the surname, name, patronymic, place of residence of the candidate (candidates) to the adoptive parents, as well as surname, name, patronymic, age, place of stay (residence) of the child. The list of documents provided by the Director of the institution where the child is, after establishing contact with the child: • copy of birth certificate of the child; • copies of documents confirming that the child is adoptable; • conclusion about state of health, physical and mental development of the child; • the act of introducing the prospective adoptive parents with the child. The prospective parents have the right to: • to see the private matter of a child who may be adopted; • conduct additional medical examination of the child. STEP 6: Service to children at the place of stay (residence) of the child to the prospective adoptive parents and documents within 10 working days prepares the citizens of Ukraine, the conclusion about expediency of adoption and its correspondence to the interests of the child for presentation in court. The documents submitted to the court: • a statement to the court about the adoption; • a conclusion about expediency of adoption; • certificate of inspection of living conditions of adoptive parents; • a document that confirms the availability of accommodation; • salary certificates; • copy of marriage certificate; • conclusion about state of health of each applicant; • information about the presence or absence of criminal record; • copy of birth certificate of the child; • one of the documents confirming the status of the child: • copy of the death certificate of the parents of the child; • the court's decision on deprivation of parental rights; • recognition of parents is unknown absent, incapacitated, deceased; • original notarized consent of parents, guardian or custodian of the child to their adoption; • the act was found, thrown the child; • consent of the children's institutions (and the consent of the child if she has reached an age and level of development that can make); • certificate of non-attendance of the child by parents (subject to the adoption under part 2 article 219 of the Family code of Ukraine); • conclusion about state of health of the child; the act of meeting prospective adoptive parents with the child. Adoption of children is carried out on the basis of a court decision. STEP 7: after Receiving a copy of the court decision (within 10 days from the date of the judgment), the adoptive parent must personally pick up the child from the institution or family, in which he was brought up in the presence of the representative for children upon presentation of a copy of the court decision on adoption. STEP 8: IN the bodies of registration of acts of civil status get a certificate of adoption and a new birth certificate with the entry about the child in the passport. STEP 9: Parents must register the child at the passport office at the place of residence.
