the Procedure for obtaining information from the State register is governed by section X of the Regulations on registration of individuals in the State register of physical persons - taxpayers, approved by order of the Ministry of revenues and duties of Ukraine dated 10.12.2013 No. 779 and registered in Ministry of justice of Ukraine on 26.12.2013 ¹ 2211/24743 (further – Position). A natural person for information about the sources of income received appeals in person or through an authorised person to the controlling body at its place of residence or place of income or location of another object of taxation, shall submit the document proving the identity, and the statement (Appendix 13 to the Regulation). The authorized person shall attach to the statement a power of attorney certified by a notary, to obtain information about the sources of income/revenues (after submission returned) and a copy of the identity document of the representative, and a photocopy (with clear picture) of the document proving the identity of the principal. Individuals not having permanent residence in Ukraine or temporarily reside outside of the locality of residence, income, location of another object of taxation, can seek information from the State register to any of the bodies of income and fees. The Supervisory authority within five working days from the date of application individuals provides information from the State register according to the forms set out in annexes 14 and 15 to the Position.
