the Department of social policy organizes the direction of the ATO participants of rehabilitation facilities to receive free psychological treatment. To obtain directions for the provision of rehabilitation services necessary shortnotice in the Department of social policy of Ternopil city Council (Ternopil, vul. Lysenko, 8, office No. 23, tel 04 37 22) the place of registration, and persons who resettled from the temporarily occupied territory or area of the antiterrorist operation, the actual place of residence, according to the certificate about registration of the person moves from the temporarily occupied territory of Ukraine or area of the antiterrorist operation, and complete the application. The application should include the following: - a copy of the certificate of the participant of military operations or the war disabled; - a copy of the document confirming the direct involvement in the implementation of the tasks of the antiterrorist operation in the areas of its implementation; - the passport and identification code (for verification). The Department of social policy delivers reimbursement of travel expenses for participants of the antiterrorist operation of rehabilitation facilities and back. The monetary compensation of cost of travel of participants of the antiterrorist operation rehabilitation facilities are available in the case of use in long-distance transport: * · train - the amount of actual expenses confirmed by travel documents but no more than the cost of travel in couchette/sleeping cars of the Express train, and the train of the second class train; * · automotive General use (except taxis) in the amount of actual expenses confirmed by travel documents. Travel costs transport urban and suburban transport are non-refundable. The monetary compensation of cost of travel of participants of the antiterrorist operation of rehabilitation facilities and back is assigned to the case when the appeal was received no later than 90 calendar days as specified in the travel document date of completion of travel. For reimbursement of the cost of travel to rehabilitation facilities and back the participants of the antiterrorist operation present in the bodies of social protection of the population at the place of registration or place of actual residence (stay) the following documents: 1) statement of grant of cash compensation, indicating the account opened in the Bank, and the address for sending the body of social protection of the population of the notification of the decision (email or paper); 2) copy of the passport or other document certifying the applicant's identity and place of residence; 3) the completed rehabilitation facility tear-off counterfoil directions in psychological rehabilitation, indicating the duration of stay in a rehabilitation facility; 4) originals of the travel documents confirming expenses for travel (the travel document must specify the date of travel and its cost).
