Vladimir Bogaychuk: the Main objective of health reform is to improve population health

June 8, the Verkhovna Rada of Ukraine adopted in first reading bill No. 6327 "On the state financial guarantees for providing medical services and medicines." For residents of the region on important aspects of the bill said in the final news of the week on TV channel "TTB" head of the Department of health Ternopil regional state administration Volodymyr Bogaychuk. According to him, during the years of Ukraine's independence the health system was one of the few unreformed areas of life of Ukrainians. "It is inefficient, does not perform its functions and is in critical condition. Patients have to pay for medication so the services of medical staff from his own pocket. So to start this reform is necessary, because this will give the opportunity to change the funding system and the relationship "doctor – patient", "patient – doctor" to residents of our region and Ukraine as a whole was able to feel the improvement in the quality of medical services and in relation to itself", – said Vladimir Bogaychuk. However, he noted that healthcare reform will pass in 2 stages. In the first stage of reform should be to separate primary care that the new model will be provided by the primary health care (PHC) network of outpatient clinics and midwife stations. The centers are responsible for providing basic assistance that is provided to the patient at the first treatment to the doctor and which is determined by the priority in reforming the health care system of Ukraine. Primary health care in the world practice means the primary health care of patients, not just treatment of patients. Therefore, a major role in the new system should play prevention. And in the second stage of the reform envisages the creation of hospital districts for the provision of medical care. The hospital district will consist of primary level facilities (PHC centers with a network of clinics) and secondary level institutions, which include the Advisory-diagnostic centres / units of the hospitals; hospital rehabilitation (rehab) treatment; hospital planned treatment; hospices; medical centers. And tertiary level, which will consist of General hospitals, where will be provided only for highly specialized medical care. "Healthcare reform provides for the implementation of the principle "money follows the patient", that is, the state will pay to hospitals and doctors for providing patients with specific medical services, not for the "content of the beds." Thus, for the interest of doctors in providing quality care, establishes the right of patients to freely choose a primary care physician that it can be implemented annually in September. In this case, the payment of physicians will depend on quantitative and qualitative indicators. That is, the primary care physician will receive the premium rate if the number of assigned patients exceeds the norm. Qualitative indicators include immunization coverage, and adult chest x-rays and early detection pavinna diseases (tuberculosis, Oncology, etc.) and the future direction of the patient to Uusikaupunki specialists. The doctor, therefore, will be a kind of guide in the medical system and to monitor the treatment at the secondary level", – said Vladimir Bogaychuk. Head of the Department of health Ternopil regional state administration stressed that health reform provides for the introduction of the concept of "State guaranteed package" – a certain amount of medical services and medicines, which the state undertakes to pay on a pre-established rates. "Tariffs each year will be approved by the government. What does 100% refund of the cost of medical services and drugs, the state guarantees in the case of a patient emergency medical care, such as in cases of heart attack, fracture, any life-threatening, the patient's primary care family doctors and receive incurable patients for palliative care. At the same time, we all need to realize that the first priority of health reform is to improve the health of the population, ensure equal and equitable access of all citizens to medical services of adequate quality. That is why the team of the Chairman of the Ternopil regional state administration Stepan Barna, the team of the Chairman of the Ternopil regional Council Victor Ovcharuk and my team of the Department of health Ternopil regional state administration fully support the current team of the Ministry of health of Ukraine, which, in fact, proposed this concept," stressed Vladimir Bogaychuk.
