IN the Center of administrative services provides service registration/deregistration of place of residence. Term of granting of the state service is up to 3 working days. A citizen of Ukraine, a foreigner or person without citizenship who permanently or temporarily reside in Ukraine, are obliged within 30 calendar days after the de-registration of place of residence and arrival to the new place of residence to register their place of residence. Parents or other legal representatives are obliged to register the place of residence of a newborn child within three months from the date of state registration of his birth. If the person cannot apply to the registration authority, the registration may be carried out according to the address of the legal representative or representative under a power of attorney certified in the manner prescribed by law (hereinafter – the representative). Registration of residence is only one address. To register the place of residence of the person or his representative submits: – a written statement in the prescribed form; – the document in which entered data on place of residence; – receipt of payment of administration fee(in the case of registration of residence at the time of withdrawal from the previous place of residence, the administrative fee will be charged only for one service); – the documents confirming : the right to reside in housing – warrant, certificate of ownership, lease (sublease, rent), a court decision entered into legal force, on granting of the right to the check-in premises, the recognition of the right of use of premises or ownership rights to it, rights to the registration of place of residence or other documents. In the absence of such documents, registration of place of residence of the person is carried out with the consent of the owner/owners of housing, the tenant and members of his family (these documents or consent is not required when registering the place of residence of minors at registration of place of residence of the parents/one of parents or lawful representative); the right to stay or registration in a specialized social institution, institution of social services and social protection entities; military service in a military unit, the address of which is indicated at registration – certificate of service in military units, issued by the commander of the military unit (for military personnel, except for conscripts); – a military ID or certificate of registration (for nationals of subject to take on military service or are on military record); – a statement of withdrawal of registration of residence (in the case of the implementation of the registration of residence at the time of deregistration from the previous place of residence) and registration card of the person (if previous residence was in the cityIn Chernivtsi). In the case of an application by a representative of the person, in addition to these documents shall be submitted additionally: – the identity document of the representative; – the document confirming powers of the person as a representative, except when the application is submitted by the legal representatives of a minor child – parents (adoptive parents). Check places of residence of the person upon application by a legal representative with the consent of other legal representatives. In the case of registration of the place of residence of the parents at different addresses, the child's place of residence, under the age of 14 shall be recorded together with one of parents with written consent of the other parent in the presence of the person making the statement, or on the basis of a duly certified written consent of the other parent (except where the place of residence of the child established by court decision or by the decision of body of guardianship and trusteeship). A person who has attained 14 years of age, freely chooses a place of residence, with the exception of restrictions established by law. Information about registration of place of residence entered in the passport of the citizen of Ukraine, temporary certificate of citizen of Ukraine, certificates for permanent residence, residence permit, refugee certificate, certificate of person in need of complementary protection, certificate of person who provided temporary protection. Children under 16 years of age and to citizens who have the passport of citizen of Ukraine in the form of a card, is issued a certificate of registration of residence. For the registration, withdrawal of registration of residence is charged an administrative fee.
